Adjusted Clinical Groups

Windrush surgery will soon be taking part in the national segmentation initiative.

This means that the practice will group together patients with similar care needs.

The groups will each have a colour and a number and this will be displayed on each patients record.

The groups help the practice to deliver services efficiently.

Patients do not need to do anything.

In the coming days you may see the group appear on your record.

There are 3 groups with 11 levels of complexity

Green – Low need group, levels 1-4

Amber – Medium need group, levels 5-9

Red – High need group, levels 10-11
